The requirements for an application for a visa for a stay of up to 90 days

(1) Unless specified otherwise below, together with the application for a visa for a stay of up to 90 days,  a foreign national  is required to submit the following upon being so requested by the authorities granting the visa:

a)a travel document;
b)photographs; this does not apply if a video image of the foreigner will be obtained;
c)an invitation confirmed by the police or proof of sufficient resources for the stay in the territory (Section 13) and a document confirming that accommodations have been secured for the foreigner for the period of the stay in the territory, unless the foreigner proves that the expenses connected with the stay in the territory have been secured in some other manner;
d)a document proving that the foreigner has sufficient means to cover expenses related to departure from the territory (Section 12);
e)a physician’s confirmation that the foreigner is not suffering from a critical disease; this confirmation can only be requested if there is a reasonable suspicion that the foreigner is suffering from a critical disease; and
f)other documents that provide proof of the information provided in the visa application.

(2) Together with the application for a visa for a stay of up to 90 days for the purpose of employment, a foreigner is required to submit a work permit issued by the Employment Office of the Czech Republic; the items specified in Subsections 1(a), 1(b), 1(d) and 1(e); and a document confirming that accommodations have been secured for the foreigner for the period of the stay in the territory. The foreigner does not have to submit a work permit if, pursuant to special legal regulations, it is not a condition for performing employment activities.

(3) Together with the application for a visa for a stay of up to 90 days for the purpose of research, a foreigner who holds a residence permit for the purposes of scientific research issued for the territory of another EU Member State is required to submit the hosting agreement concluded with the research organization registered in the applicable member state together with the required items specified in Subsections 1(a), 1(b) and 1(e) if so requested.

(4) Prior to the time that a visa for a stay of up to 90 days is granted, the foreigner is required to submit proof of health insurance for the period of the stay in the territory; this does not apply in the cases specified in Section 180i(2). If the foreigner requests the visa in the form of a multiple-entry visa, the foreigner is also required to submit an honourable declaration that a health insurance policy will be concluded for each additional stay in the territory.
