As of January 1st, 2021, the old application form (pdf, 819 kB) can only be used by EU citizens and their family members to apply for a prolongation of permanent residence permit card (Permanent Residence Card), further to apply for a new permanent residence permit card (Permanent Residence Card) replacing lost, stolen or damaged permanent residence permit card (Permanent Residence Card) or to apply for a change to be made in permanent residence permit card (Permanent Residence Card).
